Our Guide to NDIS support for yourself or your loved ones

New to the NDIS?

Hearten Community Care is dedicated to guiding you through your NDIS journey.  Our team is trained and qualified in disability support. We will work with you to provide personalised support tailored to your needs and goals.

What is the NDIS?

The NDIS offers funding to eligible individuals with disabilities by enabling access to new opportunities such as skill development, employment, or volunteering, and an overall improved quality of life. 


The NDIS facilitates connections to a wide range of community services, including healthcare professionals, community groups, sports clubs, support networks, educational institutions and libraries, as well as providing information about what support is available by each state and territory government. 

Are you eligible for ndis?

You may be eligible to receive an individual support plan if you are:

  • between the age of 9 to 65 years
  • an Australia citizen, a permanent resident, or New Zealand citizen who is a Protected Special Category Visa holder
  • have a permanent and/or significant disability that impacts your everyday living and you require assistance from others to do things
  • use equipment or assistive technology

What type of supports are funded?

 The range of supports that the NDIS may fund include:


  • daily personal activities
  • transport to enable participation in community, social, economic and daily life activities
  • workplace help to allow a participant to successfully get or keep employment in the open or supported labour market
  • therapeutic supports including behaviour support
  • help with household tasks to allow the participant to maintain their home environment
  • help to a participant by skilled personnel in aids or equipment assessment, set up and training
  • home modification design and construction
  • mobility equipment, and
  • vehicle modifications.

Reasonable and Necessary supports

The NDIS funds a range of supports and services which may include assistance in personal care, community services, employment, living arrangements and health and wellbeing.

 

In order to be considered reasonable and necessary, a support or service must be related to your disability, must not include day to day living costs not related to your disability support needs,  should represent value for money and cannot replicate support that your family or community may already assist you with.
